Study results currently being presented at the ESMO Immuno-Oncology Congress (held in London, England, December 10-12, 2025) showcase the rise of neoadjuvant immunotherapy studies "from small investigator-led window-of-opportunity...to standard of care paradigms across various tumour types." Using triple-negative breast cancer and early-stage non-small cell lung cancer as examples, treating patients before surgery provides "access to living tumour ecosystems responding - or not- to immunotherapy."

Neoadjuvant immunotherapy is valuable not only for response before surgery, but for what it teaches about anticancer immunity while the tumour is still in place. That window can reveal which immune patterns predict durable benefit and which patients are only exposed to toxicity. The oncology brothers discussions often highlight this point well: moving immunotherapy earlier is as much a biology lesson as a scheduling change.
ReplyDeleteThe "access to living tumour ecosystems responding — or not — to immunotherapy" line is the whole story in one sentence — surgery removes the evidence, so the neoadjuvant window is where the biology is legible. Moving from small window-of-opportunity studies to standard-of-care paradigms across TNBC, endometrial, and lung cancer is a remarkably fast arc. The B-cell skewing work in the endometrial abstract is a nice reminder that it is not all T cells; the humoral side deserves its own spotlight.
