Findings from the phase III GIMENA ALL2820 study, focusing on "the superiority of a frontline chemotherapy-free targeted and immunotherapeutic approach over conventional treatment in adult patients with newly diagnosed Philadelphia-positive acute lymphoblastic leukemia" was recently published in the New England Journal of Medicine. The authors of the study also acknowledged the challenges in obtaining marketing authorization for patients with newly diagnosed Philadelphia-positive acute lymphoblastic leukemia, "with overall costs of this new regimen [remaining] beyond the budget of many healthcare systems."

A chemotherapy-free regimen beating imatinib plus chemotherapy in Ph-positive ALL is the kind of result that changes the default conversation for older or less fit patients, where the chemotherapy burden has always been the limiting factor. It will be interesting to see how durable these responses prove, and how sequencing changes if this becomes the frontline standard. Conversations on the two onc docs show have been circling this same shift away from chemo-heavy approaches in haematology, and results like this give that debate real weight.
ReplyDeleteA chemo-free approach beating imatinib plus chemotherapy in Ph-positive ALL is exactly the kind of result that changes the default conversation, especially for older or less fit patients where the chemotherapy burden is the limiting factor. The durability of responses and the sequencing question will be fascinating to watch as this moves toward a new frontline standard. The cost and access concern raised in the coverage is real too — a superior regimen that healthcare systems cannot afford still leaves patients behind.
